Utah law (Utah Code § 38-1a-802) prescribes its own lien waiver form. A waiver that does not follow the statutory text may be null, void, and unenforceable in Utah.

We deliberately do not offer our generic template for Utah: serving a generic form where the law imposes a statutory text would produce an invalid waiver. Utah will be added state by state, each verified against its official source.
